Ovarian torsion (OT) is when an ovary twists on its attachment to other structures, such that blood flow is decreased. Symptoms typically include pelvic pain on one side. While classically the pain is sudden in onset, this is not always the case. Other symptoms may include nausea.

Ovarian torsion most commonly presents with severe colicky pain in the corresponding iliac fossa or lower abdomen, with 57% requiring opiate analgesia at presentation. However, there is a group of patients whose symptoms are less severe and have a slower onset. Ovarian torsion is a surgical emergency requiring early recognition and prompt intervention in order to to salvage the ovary.
